In Houston, Texas, a teenage gas-station worker was catastrophically injured while repairing a flat tire on a Harris County Sheriff’s vehicle. During the repair, a can of “tire fixer”/inflator released a flammable butane/propane mixture that allegedly ignited from a spark, triggering a flash fire and a violent tire explosion that lifted and threw the teen more than 15 feet, leaving him with multiple broken bones. The DeputySheriff that asked our client to fix a flat, also sustained a serious knee injury requiring surgery. A defective fuel tank suddenly failed, dousing a driver with gasoline vapors that ignited and engulfed him in flames. Despite extensive medical care, he died days later. The investigation revealed critical flaws in tank construction and pressure tolerance.
A young construction worker suffered severe burns when a sudden flash fire erupted inside a pickup truck during routine work travel. Investigators concluded that flammable vapors had accumulated in the cab from an improperly stored or leaking fuel/solvent container (or similar volatile product), and an everyday ignition source—such as a cigarette lighter, power-tool battery, phone charger, or electrical switch—triggered rapid ignition. The fireball spread instantly through the confined interior, leaving little time to escape and causing extensive burn injuries. The incident illustrates a recurring pattern in product and premises cases: inadequate warnings about vapor hazards in enclosed vehicles, defective or failure-prone containers/caps, and foreseeable ignition sources that make “cab flash fires” both predictable and preventable.
Each of these investigations revealed the same pattern—corporate decisions valuing cost over safety—and reinforced the need for greater oversight and engineering accountability.
